The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: (a) On or about 8/16/2017 - at 870 Vestalake Ridge Drive, Vestavia, AL, an employee was exposed to fall hazards while improperly using a body positioning belt with the lanyard attachment point located at the wearer's waist. The employees were on a 12:12 pitched roof with a fall height of approximately 34 feet from the peak to the lower level.
