29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around each hole: On or about August 21, 2017, at a bridge reconstruction site located at I-44/I-244 interchange in Tulsa, OK, an employee was working in close proximity to uncovered holes in the bridge surface, exposing the employee to the hazard of falls.
