29 CFR 1910.178(m)(5)(i): When a powered industrial truck is left unattended, load engaging means were not fully lowered, controls were not netralized, power was not shut off, and brakes set. On or about August 17, 2017 at 136 Candle Wood Road, McDonough, Georgia, an rough terrain forklift was left unattended with the load in the air.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about August 17, 2017 at 136 Candle Wood Road, McDonough, Georgia, two employees installing trusses were not using a fall protection system. The employees were exposed to a fall of approximately 40 feet to the lower level.
