29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:       a) On or about August 22, 2017, employees were exposed to the hazard of falling approximately 20 feet between two roof decks (an upper roof deck and a lower roof deck) while hoisting materials at the roof edge.     b) On or about August 22, 2017, employees were exposed to the hazard of falling approximately 20 feet between a roof deck and the ground while hoisting materials at the roof edge.         In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
