29 CFR 1910.133(a)(3): The employer did not ensure that employees wearing prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design or wears eye protection that can be worn over the prescription lenses.  a) Production Department - On or about 8/21/2017, employees were exposed to eye injuries when employer did not provide eye protection that can be worn over the prescription lenses when employees were working with hazardous chemicals such as, but not limited to,  Performance Plus Pink Dish Soap and Citric Acid Anhydrous.

29 CFR 1910.1200(f)(6)(ii): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  a) Production Department/Mixing Chemicals Area - On or about 8/21/2017, employees were exposed to skin burns, serious eye damage and respiratory irritation when employer did not provide labels for words, pictures and symbols of product identifier and general information regarding the hazards of chemicals on mixing tanks when employees were working with hazardous chemicals such as, but not limited to, LERACID K-MS 10 (corrosive) and LERASEPT PAA.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  a) Throughout facility - On or about 8/21/2017, employees were exposed to respiratory irritation when employer did not provide a written respiratory protection program that included specific procedures, fit testing, medical evaluation and training to employees required to wear full facepiece respirators when employees were working hazardous chemicals such as, but not limited to, LERACID K-MS 10 (corrosive) and LERASEPT PAA.
