29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration; such activities were not performed only by experienced and trained employees selected for such work by the competent person:  (a) At the site, employees who were performing stucco work were exposed to a 15' fall hazard, in that, the controlling contractor, Faith Plastering & Stucco, failed to ensure that the erection of the scaffold was supervised by a person trained as a competent person to do so on 8/22/2017.

29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than one inch:  a) At the site employees were performing stucco work while exposed to a 15ft fall hazard on a scaffold that was missing planks on 8/22/2017.
