29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them.)  While performing framing operations the employer did not initiate an adequate safety program that would ensure a safe and healthful working environment. In this case the employee/s were exposed to a fall hazard in excess of twelve (12) feet and a shock hazard.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  In the interior of the building being constructed a 4x6 inch blue plastic electrical box with triple duplex receptacles installed was attached to the end of the black two foot flexible cord. This box was being used to energize portable power tools and  was not listed, labeled or certified for use as a part of an extension cord/distribution box. Employees were exposed to the hazard of contacting an energized electrical circuit resulting in a shock.a shock hazard.

29 CFR 1926.405(b)(2): All pull boxes, junction boxes, and fittings were not provided with covers. In energized installations each outlet box did not have a cover, faceplate, or fixture canopy.   In the interior of the building being constructed a 4x6 inch blue plastic electrical box with triple duplex receptacles installed did not have a cover installed that adequately/completely covered the open areas of the box. Employees were exposed to the hazard of contacting an energized electrical circuit resulting in a shock.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  In the interior of the building being constructed a 4x6 inch blue plastic electrical box with triple duplex receptacles installed was attached to the end of the black two foot flexible cord. The black cord did not have the outer insulation of the cord attached to the box in a manner to ensure that any strain would not be transmitted to the screws within the box. Employees were exposed to the hazard of contacting an energized electrical circuit resulting in a shock.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  The employer did not ensure that the ladder being utilized to access the roof where roof sheeting was being installed was extended at least three feet above the landing surface of the roof. It was extended approximately 18 inches. The employee was exposed to a fall hazard of approximately one (1) foot from the eave to the ground below.
