29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. a. On or about August 21, 2017, employees engaged in construction activities at the above referenced worksite were working in areas requiring personal protective equipment to safeguard against falling, flying debris, and overhead hazards associated with aerial lift operations. Employees were thereby exposed to struck-by hazards likely to cause head injuries. There is no abatement certification of documentation required for this item.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. a. On or about August 21, 2017 at the above addressed jobsite, employees working in an aerial lift were not properly attached to the boom or basket by the use of a lanyard, thereby exposing the employees to the hazards associated with a fall of more than fifteen (15) feet. There is no abatement certification of documentation required for this item.
