29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.    100 Bobala Rd, Holyoke, MA    On August 22, 2017 and at times prior, the employer's energy control procedures were not accurate for some machines, including and not limited to the Amada with ASR machine.  Also, many energy control procedures erroneously included stop, start, and emergency stop switches as lockout locations.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    100 Bobala Rd, Holyoke, MA    On August 22, 2017, the employer had only performed a 2015 accuracy review of each energy control procedure.  The employer's procedure for annual inspection did not meet three requirements of this standard: 1) did not inspect each procedure for inaccuracies during an actual energy control maintenance or servicing task, 2) did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under each energy control procedure, and 3) did not require the inspection to be performed by an authorized employee other than the ones utilizing the energy control procedure.
