29 CFR 1910.212(a)(3)(i): One or more methods of machine guarding was not provided to protect the operator at the point of operation on a machine where work was actually performed upon the material being processed: (a) Preform Department: On or about August 22, 2017 the employer did not provide machine guarding to protect employees from the drill bit on a Fehlmann Milling Machine (No. 23) in that an employee's fingers are as close as one-quarter inch from the rotating bit while holding the piece that is being drilled.

A 3 inch by 11 inch opening was not guarded exposing employees to a moving 1/2 inch belt.
