29 CFR 1910.25(b)(3): The employer did not ensure that the stairs had uniform riser heights and tread depths between the landings: a) At the maintenance lube pit: On or about August 21, 2017, and at times prior thereto, the employer did not ensure uniformity of stairway riser heights between landings, exposing employees to hazards not limited to slips and falls as they accessed the pit to perform maintenance activities.

29 CFR 1910.28(b)(8)(ii): Floor markings at least 6 feet (1.8 m) from the edge of the pit were not in colors that contrast with the surrounding area and a warning line at least 6 feet (1.8 m) from the edge of the pit as well as stanchions were not in place: a) At the maintenance lube pit: On or about August 21, 2017, and at times prior thereto, the employer did not ensure that warning lines and stanchion chain were at least 6 ft. from unprotected edges, exposing employees to 5-ft. falls into the pit and down stairs as employees frequented the area while performing maintenance activities.
