29 CFR 1910.178(l)(3)(ii)(C): Workplace-related topics: Load manipulation, stacking, and unstacking; On 8/22/2017, prior to moving the Twin Spindle CNC machine, employees were not trained on load manipulation, stacking and unstacking for the Twin Spindle CNC machine located at Walther Engineering located at 3501 Shotwell Dr., Franklin, OH in that the CNC machine weighed approximately 37,840 pounds and the Versa Lift forklift being used had a maximum capacity of 35,000 pounds. Three employees were attempting to move the machine from a corner at the facility by placing part of the load on the forks to guide the machine out of the corner.

29 CFR 1910.178(m)(2): No person shall be allowed to stand or pass under the elevated portion of any truck, whether loaded or empty. On 8/22/2017, an employee was located under the elevated portion of the Versa Lift 25/35 to place wooden blocks under the load (Twin Spindle CNC machine weighing approximately 37,840 pounds) to stabilize the load. The load became unstable while the employee was under the elevated portion of the truck and fell onto the employee.
