On or about August 23, 2017, at the above addressed jobsite, an employee was not trained in the recognition of jobsite hazards, which included but not limited to personal protective equipment, pneumatic tools, and hand tools.

On or about August 23, 2017, at the above addressed jobsite, employees conducting roofing operations were not provided training on how to minimize fall protection hazards, thereby exposing employees to the hazards associated with falls.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a.  On or about August 23, 2017, at the above addressed jobsite, employees performing residential roofing operations on a steep sloped roof (5:12 or above), twenty (20) feet above the ground were not protected from falls, thereby exposing employees to fall hazards.    Peak Construction Services Corp. was previously cited for a violation of an equivalent Occupational Safety and Health Standard  29CFR 1926.501(b)(10) which was contained in OSHA inspection number 1097235, citation number 1, item number 1 and was affirmed as a final order on April 6, 2016, with respect to a workplace located at 11100 S. Indiana Ave., Chicago IL 60605.
