On or about August 23, 2017, at the above addressed jobsite, employees performing residential roofing operations on a steep sloped roof (5:12 or above), twenty (20) feet above the ground were not protected from falls, thereby exposing employees to fall hazards. Peak Construction Services Corp. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29CFR 1926.501(b)(10) which was contained in OSHA inspection number 1097235, citation number 1, item number 1 and was affirmed as a final order on April 6, 2016, with respect to a workplace located at 11100 S. Indiana Ave., Chicago IL 60605.
