29 CFR 1910.147(c)(7)(i)(C): All other employees whose work operations are or may be in an area where energy control procedures may be utilized, shall be instructed about the procedure, and about the prohibition relating to attempts to restart or reenergize machines or equipment which are locked out or tagged out: a) Sizzling Platter dba Little Caesars, 2960 Federal Blvd, Unit 9, Denver, CO: On and prior to August 19, 2017, the employer did not ensure that all other employees whose work operations are or may be in an area where energy control procedures may be utilized, were instructed about the procedure, and about the prohibition relating to attempts to restart or reenergize machines or equipment which are locked or tagged out. An employee experienced a finger amputation when cleaning the Somerset dough sheeter machine and coming to contact with the rotating nip point. This condition exposed the employee to an amputation hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: (a) Sizzling Platter DBA Little Caesars, 2960 Federal Blvd, Unit 9, Denver, CO: On and prior to August 24, 2017, the employer did not maintain an adequate strain connection between the 120 volt flexible cord and plug on the Somerset dough sheeter machine to prevent pull from being directly transmitted to the terminal screws. A visual inspection revealed the outer sheath on the flexible cord was pulled away from the plug.
