29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about August 23, 2017, at a worksite located at 8190 Leatherman, Wadsworth, Ohio 44281, during residential framing activities, the employer did not ensure employees were protected from foreign body eye hazards by ensuring eye protection was available and worn during nailing, cutting and material installation activities. Byler Carpentry, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1188301 citation number 1, item number 2 and was affirmed as a final order on 12/27/2016, with respect to a workplace located at 1676 Par Four CR, Massillon, Ohio 44646.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about August 23, 2017, employees were exposed to a fall of approximately 13 feet. The employees were performing framing activities on a new residential home located at a worksite located at 8190 Leatherman, Wadsworth, Ohio 44281. The employees were working on all areas of the 2nd level without using any form of fall protection. Byler Carpentry, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1188301 citation number 1, item number 3 and was affirmed as a final order on 12/27/2016, with respect to a workplace located at 1676 Par Four CR, Massillon, Ohio 44646.
