29 CFR 1910.178(l)(3)(i)(M): The employer did not review the operating instructions, warnings, or precautions listed in the operator's manual for the types of vehicle that the employee is being trained to operate:  At the location of 19615 W 65th St. in Shawnee KS, an employee engaged in lifting material to other employees with the use  of an all-terrain forklift (Model 8042) and workers nearby were exposed to crush and struck by hazards, in that the seatbelt was not in use in accordance to the JLG Operation and Safety Manuel Models 8042 & 10054 in Section 1-General Safety Practices.
