29 CFR 1926.1053(b)(6): Ladders were used on surfaces which were not stable and level:    a) 24 Bennett Street, Manchester-By-The-Sea, MA 01944    On or about August 23, 2017, the employer did not ensure that ladders used on surfaces that were not stable and level were secured against falling. An employee using an a-frame ladder on a pitched roof fell from the ladder and the roof to a lower level, approximately 12 feet, severely injuring the employee.     There is no abatement certification or documentation required for this item.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:    a) 24 Bennett Street, Manchester-By-The-Sea, MA 01944      On or about August 23, 2017, the employer did not ensure that employees were adequately trained on using ladders, which would enable each employee to recognize hazards related to ladders, and train each employee in the procedures to be followed to minimized these hazards. Employees, one of which was previously trained on ladder safety, did not recognize the hazard of the ladder on the pitched roof without adequate securing. An employee using an unsecured a-frame ladder on a pitched roof fell from the ladder and the roof to a lower level, approximately 12 feet, severely injuring the employee.     Reynolds Seamless Gutter, Inc. was previously cited for a substantially similar violation of the Occupational Safety and Health standard, or it's equivalent standard 29 CFR 1926.1060(a) which was contained in OSHA inspection number 914881, citation number 1, item number 1, issued on July 31, 2013, and became a final order on September 12, 2013.     Abatement documentation is required for this item in accordance with the requirement of 29 CFR 1903.19(d).
