29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee: a) The Village at Northgate, Lot 23, Worksite - on or about August 23, 2017, two employees were working from a scaffold platform lifted to a working height of approximately 19 feet, 2 inches above grade Both employees were wearing personal fall arrest harnesses and lanyards that were attached to a single anchorage strap which was only rated for a single worker's maximum arresting force of 5000 pounds.

29 CFR 1926.451(a)(6): Scaffolds were not loaded in accordance with the design by a qualified person: a) The Village at Northgate, Lot 23, Worksite - on or about August 23, 2017, an engineered scaffold platform was lifted to a working height of approximately 19 feet, 2 inches above grade by a powered industrial truck, where the total length of the platform extended beyond each side of the forklift's wheel base by approximately 5 feet, which is not accordance with its design requirements.
