29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) At the south side of the building where gutters where being installed with the use of a JLG aerial lift, model M600AJ, serial number 0300214872, no fall protection was provided exposing two employees to a 20 ft. fall hazard on or about 8/17/2017.
