(a) Ruiz Tree & Landscape Services, LLC on or about August 23, 2017, at the address of 3925 Central Ave. Toledo, OH: The employer did not ensure an employee performing tree trimming/removal operations from an elevated vehicle mounted platform, utilized an appropriate fall protection system. The employee performing work in this condition was exposed to a fall up to approximately 50 ft. to the ground below.

Employees performing tree removal activities without an employer assessment were exposed to eye, head and bodily injuries.

29 CFR 1910.180(d)(3)(iii): Frequent inspections were not conducted to identify whether safety devices were fully functioning.   (a)  Ruiz Tree & Landscape Services, LLC on or about August 23, 2017, at the address of 3925 Central Ave. Toledo, OH: The employer did not ensure frequent inspections of safety devices were performed on the National Crane model 556A truck mounted crane. The crane's anti-two block system was inoperable/removed, exposing employees working on or around the load to struck-by hazards.
