29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a. On or about August 15, 2017, parking lot driveway, an employee in the bucket of an aerial lift truck was within proximity of an overhead power line that was not de-energized or guarded by insulation which exposes employees to electrical hazards.

29 CFR 1926.416(a)(3): Employer did not advise employees of the location of energized electrical power circuits, the hazards involved, and the protective measures to be taken: a. On or about August 15, 2017, parking lot driveway, the employer did not advise employees on adequate protective measures including the minimum safe approach distance to be maintained from energized overhead power lines. Employees worked at an aerial lift truck and the bucket was within proximity of an energized overhead power line which exposes employees to electrical hazards.
