On or about August 17, 2017, at the hotel parking lot, an employee worked from a platform that was supported by the forks of a rough terrain forklift truck and the platform was not constructed in accordance with ANSI/ITSDF 56.6 which exposes the employee to struck-by and fall hazards.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:    a.  On or about August 17, 2017, at the hotel parking lot, an employee worked from a platform that was supported by a rough terrain forklift truck.  The platform was not approved for use by the forklift truck manufacturer and exposes the employee to struck-by and fall hazards.

29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks.    a.  On or about August 17, 2017, at the hotel parking lot, forklift truck operations did not meet operation requirements of ASME B56.1-1969 in that the forklift truck was not attended by the forklift operator when the forks were elevated and exposes employees to struck-by and fall hazards.
