On or about August 17, 2017, at the hotel parking lot, an employee worked from a platform that was supported by a rough terrain forklift truck. The platform was not approved for use by the forklift truck manufacturer and exposes the employee to struck-by and fall hazards.

On or about August 17, 2017, at the hotel parking lot, forklift truck operations did not meet operation requirements of ASME B56.1-1969 in that the forklift truck was not attended by the forklift operator when the forks were elevated and exposes employees to struck-by and fall hazards.
