29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    a. On or about August 23, 2017 one employee was not wearing appropriate eye protection while using pneumatic nail guns while installing new roof shingles exposing him to eye injury due to impacts from debris or dirt associated with using the nail gun.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a.   On or about August 23, 2017 four employees were installing new roof shingles on a 2 story, 5:12 steep slope multi residential structure and were not protected by any means of personal fall protection, guardrails or safety nets, exposing them to falls in excess of 19 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a. On or about August 23, 2017 employees were using a Werner Brand Aluminum Extension ladder to gain access to the roof and were not protected from falling while climbing on or off the roof as the ladder side rails did not provide an adequate hand hold because the side rails did not extend at least 3 feet above the upper landing surface.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a. On or about August 23, 2017 CSHO observed 4 employees working on the roof of a two story multiple resident structure and were not protected by any means of personal fall protection, guardrails or safety nets, exposing the employees to falling in excess of 19 feet to the ground below. The employees told CSHO they were not provided with Fall Protection training from the employer Lewis Walker Roofing, Inc.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  a. On or about August 23, 2017 employees were using a ladder to gain access to the second story roof top and did not place or set the ladder with the side rails extending 3 feet above the upper landing surface. The ladder was not secured to the roof to prevent slipping or displacement while climbing the ladder.
