29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) Roof - An employee installing PVC trim on the edge of a split level roof was exposed to a fall hazard of approximately twenty (20) feet without the protection of a guardrail system, safety net system, personal fall arrest systems or an alternate fall protection measure, on or about August 21st, 2017.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) Roof - An employee using a twenty-four (24) foot extension ladder to access and egress from the roof was exposed to a fall hazard of approximately twenty (20) foot to the ground without the ladder rails extending at least three (3) feet above the working surface, on or about August 21st, 2017.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  a) Roof - An employee using a twenty-four (24) foot extension ladder to access and egress from the roof was exposed to a fall hazard of approximately twenty (20) foot to the ground without training to recognize the hazards of a ladder that was not setup properly, on or about August 21st, 2017  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
