29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On or about August 24, 2017:    a) Employees removing and installing shingle material on a residential steep roof were exposed to fall hazards of approximately 10 feet, when the employer failed to ensure that each employee received and used proper fall protection measures.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about August 24, 2017:    a) Employees engaged in roofing work to include removing and installing shingle material were exposed to fall hazards of approximately 10 feet at the eave of the house, when the employer failed to properly train employees in the recognition, reduction or elimination of the fall hazard they were exposed to.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.1053(b)(7): Ladder(s) that were unsecured or not provided with slip-resistant feet were used on slippery surfaces:    On or about August 24, 2017:    a) Employees utilizing an extension ladder to gain access to the upper working surface were exposed to fall hazards of approximately 10 feet, when the ladder they were using was place on a sloped driveway surface without any slip resistant feet attached.     No abatement certification or documentation is required for this item.
