29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about August 23, 2017, at Lot 20b, 1975 Township Drive, Auburn, Georgia:  a) Two employees performing roofing work were exposed to a fall hazard of approximately 17-feet and greater when they worked from an approximately 10/12 slope roof without utilizing fall protection.  b) An employee performing roofing work was exposed to a fall hazard of approximately 17-feet when he worked from the gables of the house using a carabiner to connect directly to the webbing of a rock climbing harness at the front of his waist.  c) An employee performing roofing work was exposed to a fall hazard of approximately 13-feet when he worked from the garage roof of the house without using fall protection.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  On or about August 23, 2017, at Lot 20b, 1975 Township Drive, Auburn, Georgia:  An employee performing roofing work was exposed to a fall hazard of approximately 17-feet when he and his supervisor used a damaged Werner 28-foot extension ladder with broken rungs at the same time which was designed for single access/egress.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder:  On or about August 23, 2017, at Lot 20b, 1975 Township Drive, Auburn, Georgia:  a) Two employees performing roofing work were exposed to a fall hazard of approximately 17-feet when they used an aluminum extension ladder that was set with the base of the ladder set approximately 12-feet from top support of the ladder.   b) Two employees performing roofing work were exposed to a fall hazard of approximately 10.3-feet when they used an aluminum extension ladder that was set with the base of the ladder set approximately 6-feet from top support of the ladder.

On or about August 23, 2017, at Lot 20b, 1975 Township Drive, Auburn, Georgia:  Two employees performing roofing work were exposed to a fall hazard of approximately 17-feet when they used an aluminum extension ladder that was damaged with broken rungs.
