On or about August 19, 2017 employees were exposed to fall hazards and the employer did not initiate and maintain an accident prevention program to control and eliminate the hazards present at the jobsite in accordance with the OSHA regulations.

29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On or about August 19, 2017 employees performing construction activities were exposed to fall hazards and a competent person had not performed frequent and regular safety inspections of the construction site to identify and eliminate the hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about August 19, 2017 employees were exposed to fall hazards while installing roof trusses because none of the fall protection systems specified in this section were used to protect employees from falling approximately 20 feet to the ground below.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. On or about August 19, 2017 employees performing construction activities and installing trusses, approximately 20 feet from the ground below, were exposed to fall hazards because the employees were not provided training which would have enabled them to recognize the hazards of falling and provide them with the procedures to follow to minimize the hazards.
