29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about August 23, 2017, at the above addressed jobsite, employees were on a residential roof walking and working on the roof to access a hatch to spray insulation into the attic. Employees were not utilizing fall protection while on the roof. Employees were exposed to fall hazards of approximately 8.9-17.8 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: On or about August 23, 2017, at the above addressed jobsite, employees were not provided with a safe means of access to the main roof of the home. Employees were accessing the roof of the home by standing on materials on the roof of the garage exposing employees to fall hazards. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
