29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection.  This most recently occurred on August 22, 2017 on the jobsite located at 608 Fleetwood Dr., Papillion, NE 68133.  Four employees were observed on the roof of a 2-story house with 6 in 12 and 10 in 12 steep sloped roofs at heights greater than 6 feet above the ground and not utilizing any form of fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   The employer is failing to ensure that each employee exposed to fall hazards has been effectively trained on fall hazard recognition and on how to use fall protection.  This most recently occurred on August 22, 2017 on the jobsite located at 608 Fleetwood Dr., Papillion, NE 68133.  The employer has 4 employees who have not receive fall protection training and are exposed to fall hazards while re-roofing an existing 2-story residential home that has 6 in 12 and 10 in 12 steep sloped roofs and a ground to eave height of about 11 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   The employer is failing to ensure that all employees are protected from fall hazards when entering and exiting the landing surface of a residential roof.  This most recently occurred on August 22, 2017 on the jobsite located at 608 Fleetwood Dr., Papillion, NE 68133.  Employees are an aluminum extension ladders to enter and exit the roof where the side rails were not extended to at least 3 feet above the landing surface which is about 11 feet above the ground.

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways:   The employer is failing to train the employees to recognized fall hazards associated with using a ladder to access and egress a roof of an existing residential home.  This most recently occurred on August 22, 2017 on the jobsite located at 608 Fleetwood Dr., Papillion, NE 68133.  The employer had untrained employees using an aluminum extension ladder as a means to access and egress the roof which has an eave to ground height of approximately 11 feet.  The ladder being used was not extended a minimum of 3 feet over the upper landing surface.
