29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions:    East side of building 8:  On or about August 21, 2017, employees were exposed to a chemical hazard while applying Millennium PG-1, Pump Grade Adhesive without wearing adequate personal protective equipment such as, but not limited to, eye protection, impermeable gloves, and body protection.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    Roof of building 8:  On or about August 21, 2017, employees were exposed to a fall hazard greater than 6 feet to the concrete ground below while using a warning line system without having a designated safety monitor present.

East side of building 8:  On or about August 21, 2017, employees were exposed to a struck by hazard while the operator of a JLG forklift, Model 69-43A, left the forklift unattended with the load raised.
