29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:  location: exterior of building on or about: 8/22/17  a) Employees were working from a tubular welded frame scaffold. The working levels of the scaffold were not fully decked. Employees were exposed to a fall of up to 26 feet.  Note: In addition to abatement certification, the employer is required to provide abatement documentation for this item in accordance with 29 CFR 1903.19

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level:  location: exterior of building on or about: 8/23/17  a) Employees, engaged in stucco and stonework from a tubular welded frame scaffold, were not provided with fall protection. The employees were exposed to a fall of up to 26 feet.  Note: In addition to abatement certification, the employer is required to provide abatement documentation for this item in accordance with 29 CFR 1903.19

29 CFR 1926.454(a): The employer shall have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  location: entire site on or about: 8/23/17  a) Employees were working from a tubular welded frame scaffold. One of the employees was not provided with scaffold safety training.   NOTE: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
