This most recently occurred on August 24, 2017 on the jobsite located at 2502 Alexandra Rd, Papillion, NE 68133. Employees were observed using a hand-held pneumatic nail gun to install roofing material without any form of eye or face protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection. This most recently occurred on August 24, 2017 on the jobsite located at 2502 Alexandra Rd, Papillion, NE 68133. Employees were observed on the roof of an existing ranch-style house with an 8 in 12 steep sloped pitch at heights greater than 6 feet above the ground and not utilizing any form of fall protection.
