29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. On or about August 25, 2017, at a new structure construction site located at 600 E. Kenosha in Broken Arrow, OK, employees installing metal roofing material were allowed to work on a roof without fall protection, exposing the employees to the hazard of falls.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: On or about August 25, 2017, at a new structure construction site located at 600 E. Kenosha in Broken Arrow, OK, employees were allowed to use a ladder that did not extend at least 3 feet beyond the top of a roof, exposing the employees to fall hazards due to ladder displacement.
