29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly: Plant - On or about 08 /24/17 and at times prior thereto the employer did not inspect and remove from service a fire extinguisher that exhibited corrosion damage. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.305(d)(2): Panelboard enclosures. Panelboards other than the dead front externally-operable type were mounted inside cabinets and were accessible to unqualified persons. a) Plant, Packing Room - On or about 08/24/17 and at times prior thereto unqualified operators accessed the printers/clocks placed inside the cabinets (which were not dead-front) , and were exposed to accidental contact with 240 V live parts. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
