29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: a) Manufacturing Area - On or about August 25, 2017 and at times prior, employees were exposed to respiratory hazards in that the were provided tight fitting 3M respirators but were not given the information contained in Appendix D of the Respiratory Protection standard.

29 CFR 1910.1200(b)(1): The employer failed to provide HAZCOM training in accordance to their written HAZCOM program. a) Manufacturing Plant - On or about August 25, 2017 and at times prior, employees were exposed to chemical hazards in that they are required to work with chemicals such as but not limited to PPG Urethane Hardener and PPG Slow Reducer and the company has not implemented a written HAZCOM program in that management is unaware of the existence of such a program.
