29 CFR 1904.32(b)(2)(ii): The annual summary did not include one or more of the following elements: the calendar year covered, the company's name, establishment name, establishment address, annual average number of employees covered by the OSHA 300 Log, and the total hours worked by all employees covered by the OSHA 300 Log.  a) Norfolk Naval Shipyard, Branch Health Care Clinic, Portsmouth, VA 23709:  The employer combined the recordable injuries and illnesses, number of employees and total hours worked from a Naval Medical Center Portsmouth location with the establishment at Norfolk Naval Shipyard Branch Health Care Clinic on the OSHA 300A Summary log or equivalent form.  Abatement documentation is required for this item.

29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four business hours:  a) Norfolk Naval Shipyard, Branch Health Clinic, Portsmouth, VA 23709: The OSHA 300 Logs or equivalent forms requested on August 9, 2017 were received on August 16, 2017 for 2014, 2015, 2016 on a non-equivalent form. An OSHA 300 Log was not received for 2017.   Abatement documentation is not required for this item.
