29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    Site: 656 Warwick St. Brooklyn, NY.    On or about 8/8/2017,    a) Employees working from the bulkhead of a 2 story building were not protected from falling.  Employees installiing an air conditioning unit were exposed to a fall of approximately 11 feet.  An employee fell from the bulk head to a lower roof and sustained serious injuries.

Site: 656 Warwick Street, Brooklyn, NY.    On or about 8/8/2017    a) Employer did not ensure employees exposed to fall hazards were trained.  Employees installing an air conditioning unit from the bulkhead of a 2 story building without the use of any fall protection.  An employee was seriously injured when he fell approximately 11 feet.

On or about 8/8/2017    Site: 656 Warwick Street, Brooklyn, NY.    a) Stairway leading from ground level to the roof were not filled in with concrete or other material.  Employees used the stairway daily to travel between floors and access the work area on the roof.

An employee sustained serious injuries after falling approximately 11 feet while working on a construction site.
