29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift. On 8/16/2017, An employee from Meza ProDoors Corporation was installing decorative gutters on a commercial building located at 7351 Highway N, Dardenne Prairie, MO. The employee was working from an aerial lift, which had the boom in the elevated position approximately nineteen feet above the ground. The employee was inside the boom basket working with the use of body belts or body harnesses and lanyards not attached to the boom or basket of the aerial lift.

Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. On 8/16/2017, employees from Meza Prodoors Corporation were installing decorative guttering on a commercial building located at 7351 Highway N, Dardenne Prairie, MO. Employees were on the roof which was approximately nineteen feet above the ground, without the proper use of guardrail systems, safety net systems, or personal fall arrest systems or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.
