29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system:    (a) Finish Station: On or about August 25, 2017 and at times prior, an employee was sweeping rainwater off a modular building roof at a height of approximately 15 ft. without fall protection.    (b) Wood Roofs Station: On or about August 25, 2017 and at times prior, employees work on the roofs while being connected to a lifeline with a retractable lanyard and harness at a height of approximately 15 ft.  Employees connect to lanyard and put on fall protection on the roof while exposed to fall hazards.    Note: Abatement certification is required for this item.

29 CFR 1910.30(a)(1): (a) Fall hazards. (1) Before any employee is exposed to a fall hazard, the employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart.  Employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  (a) Finish Station: On or about August 25, 2017 and at times prior, an employee was sweeping rainwater off a modular building roof at a height of approximately 15 ft.
