29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.    Location: 192 Summer Street, Kingston, MA    On or about Aug. 28, 2017 employees were exposed to struck-by hazards while working on a mobile scaffolding, without hardhats.

29 CFR 1926.452(w)(6)(i): Employee(s) were allowed to ride on scaffolds when the surface on which the scaffold was moved was not within 3 degrees of level, and/or not free of pits, holes, and obstructions    Location: 192 Summer Street Kingston, MA    On or about Aug. 28,2017 employee were exposed to fall hazards of approximately 18 feet to the asphalt below, where an obstruction caused the mobile scaffold to exceed 3 degrees of level, causing it to tip over.
