The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    Employees who performed the changing of the bandsaw blades of the Salem Headrig Bandsaw machines were not required to:    1.  Know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy.   2.  Affix lockout devices on the electrical, pneumatic energy isolating device, and the residual energy from the saw blade movement after shutdown.    3.  Prior to starting work on machines that have been locked out, verify that isolation and deenergization of the machine or equipment have been accomplished.    On August 16, 2017, at the #2 Salem Headrig Bandsaw machine, two employees were exposed to an amputation hazard while removing and installing another bandsaw blade on the machine.
