This was documented at the jobsite located at or near 456 Brockton Dr., Lee's Summit, MO. The employer did not provide or require the use of benching, sloping, shoring or other means to ensure that employees working in excavations were protected from excavation wall failure. An employee was observed working in a trench approximately 5.4 feet in depth without any protective systems.
