29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    On or about August 25, 2017, at the intersection of Jodeco Road and Peach Drive, Stockbridge, Georgia, while installing a new water line and taking measurements and checking alignment in a trench approximately eight feet deep, to egress the trench an employees had to walk more than 25 feet down the length of the trench to access the ladder.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.      On or about August 17, 2017 at the intersection of Jodeco Road and Pac Drive, Stockbridge, Georgia: an employee working in a unprotected trench was exposed to a cave in hazard when the spoil pile was placed on the edge of the trench.

29 CFR 1926.651(k)(2): Where the competent person found evidence of a situation that could result in a possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employees were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety      On or about August 17, 2017 at the intersection of Jedeco Road and Peach Drive, Stockbridge, Georgia: where the competent person was aware of the existing hazardous conditions on site, an employee was allowed to work in an unprotected trench approximately 8 feet deep. The employees were exposed to the following hazards, to include, but not limited to the following:     a. employee was exposed to a cave in hazard when the spoil pile was placed on the edge of the trench    b. a protective system was not used to protect the employees working in the trench from cave-in

On or about August 17, 2017 at the intersection of Jedeco Road and Peach Drive, two employees checking alignment for the installation and connection of a new water line were working in an unprotected trench approximately 8 feet deep.
