29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a) On or about 08/16/2017 - 285 Silver Creek Parkway, Alabaster, Alabama, employees were exposed to fall hazards while removing and installing shingles on a steep roof with an 8:12 pitch at approximately 10 feet 6 inches above ground level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about 08/16/2017 - 285 Silver Creek Parkway, Alabaster, Alabama , employees were using a Werner aluminum ladder to access the roof of a single family house that was not properly extended.
