(b) New townhome construction located on Lot #7, Roof deck: On or about 08/28/2017, and at times prior, the employer exposed employees to fall hazards of over 37-feet while working on a 12/12 pitch roof, in that a complete fall protection system was not provided or used by employees installing plywood sheathing.
