29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Rig #44, New Milford, PA: A relocatable power tap (surge protector) installed in the doghouse that was powered by an extension cord and used to provide electrical power to a small refrigerator, a coffee maker, and two battery chargers was not installed and used in accordance with the devices approved use exposing workers to a fire and electric shock hazard, on or about August 28, 2017. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED Southwestern Energy was previously cited for a violation of this Occupational Safety and Health Administration standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 946460, citation number 1, item number 3, and was affirmed as a final order on 1/22/2014, with respect to a workplace located at Bradford County, PA.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: a) Rig #44, TNT Pad, New Milford, PA: An extension cord that was run along the ceiling of the doghouse was used to power several electronic devices in place of fixed wiring, on or about August 28, 2017. b) Rig #44, TNT Pad, new Milford, PA: A short extension cord was used in place of fixed wiring to provide power to a relocatable power tap (surge protector), on or about August 28, 2017.
