29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:  At a residential construction site located at 4104 NE Courtney Dr., Lee's Summit, MO: Employees were exposed to fall hazards. The employer failed to provided adequate fall protection for the employees as they were observed working from height without the installation of guardrails on all side and ends of the platform. Two employees were working from a pump jack scaffold at a height of approximately 35 feet to the  ground below.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)  At a residential construction site located at 4101 NE Courtney Rd., Lee's Summit, MO: Employees were exposed to fall hazards. The employer failed to provide adequate training that assures all employees exposed to fall hazards understood the hazards associated with falls and what measures are needed to protect themselves. This was demonstrated when employees were observed working at height on a jack scaffold without the use of any fall protection systems at heights of approximately 35 feet to the ground below.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed  At a residential construction site located at 4104 NE Courtney Dr., Lee's Summit, MO: Employees were exposed to fall hazards. The employer failed to protected employees from use of equipment for which it was not designed. Employees were observed working from a platform of a pump jack scaffold that was being supported on one end by a 5 foot step ladder. The step was ladder was not designed per the manufacturer for this use.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a residential construction site located at 4104 NE Courtney Dr., Lee's Summit, MO: Employees were exposed to fall hazards. The employer failed to provid safe access to employees at a residential construction site when employees were observed using portable ladders to gain access to a roof and the ladder was placed only one and one-half foot above the upper landing.
