29 CFR 1926.502(h)(1)(iv): The safety monitor was not close enough to communicate orally with the employee: (a) At the commercial construction site located at 305 US 24 Highway, Independence, Missouri: Employees engaged in commercial roofing activities on a flat roof were exposed to fall hazards greater than 25 feet from the roof of the building to the ground below in that the safety monitor was at the opposite end of the building from an employee, who stepped outside the warning line to lean over near the edge of the roof, and was not close enough to communicate orally with the employee. Fall protection was not utilized.
