An employee engaged in commercial welding activities was observed working from an aerial lift basket without a lanyard attached to the fall harness.     Violation observed on or about August 29, 2017    b. 2655 Richmond Ave, Staten Island, NY - Staten Island Mall - Between garage and mall expansion    The employer did not ensure that employees working from a JLG aerial lifts were protected from falls.  An employee engaged in commercial welding activities was observed working from an aerial lift basket without a harness and a lanyard attached to the lift basket.     Violation observed on or about August 29, 2017    JC Steel Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.453(b)(2)(v),  which was contained in OSHA inspection number 801162, citation number 1, item number 2, and was affirmed as a final order on February 21, 2014 with respect to a workplace located at 1701 Sunrise Highway, Bay Shore, NY 11706.
