29 CFR 1910.25(b)(3): The employer did not ensure that the stairs had uniform riser heights and tread depths between the landings:    a) Ceiling chamber: The employer did not ensure that the stairs leading up to the ceiling chambers had uniform riser heights. Measurements of the risers at the bottom of the stairs were 13, 12 3/4 and 12 1/2 inches, on or about 10/12/17.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.28(b)(11)(ii): The employer did not ensure each flight of stairs having at least 3 treads and at least 4 risers was equipped with stair rail system and handrails per Table D-2 Stairway Handrail Requirements:  a) Ceiling chamber Echo Division Truss 5 Mississippi Ave: The stairs leading to the ceiling chambers of the organ did not have a stair rail system with a top and mid rail on the left side ascending, on or about 9/12/17.  b) Ceiling chamber Echo Division Truss 5 Mississippi Ave: The stairs leading to the ceiling chambers of the organ did not hav a mid rail on the right side ascending, on or about 9/12/17.  c) Ceiling chamber Fanfare Division Truss 5 Georgia Ave: The stairs leading to the ceiling chambers of the organ did not have a stair rail system with a top and mid rail on the left side ascending, on or about 9/12/17.  d) Ceiling chamber Fanfare Division Truss 5 Georgia Ave: The stairs leading to the ceiling chambers of the organ did not have a mid rail on that right side ascending, on or about 9/12/17.   NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
